“Luminary Mind Wellness” is a trade name (DBA) of Vialpando Complete Care, which operates this page and is responsible for the information submitted through it.
What this page collects
The appointment request form collects your name, phone number, email address, insurance carrier, and how you would prefer to be contacted. It is used to contact you about your request.
Please do not include medical or health details in this form. It is not a secure channel, and submitting it does not create a provider–patient relationship. Anything clinical is collected securely once you are scheduled.
Forms on Facebook and Instagram
Luminary Mind Wellness may run ads on Facebook and Instagram with a request form that opens inside those apps. That form collects your name, phone number, email address, and insurance carrier, and Meta shares them with us so we can contact you about your request.
Please do not include medical or health details.
Meta’s handling of that information is covered by Meta’s Privacy Policy. To have the information deleted, call (720) 778-0513.
Advertising cookies and tags
This page uses advertising and measurement technologies from Google (Google Ads) and Meta (Facebook and Instagram). These set cookies and similar identifiers that let us understand which advertisements lead people to request an appointment.
We also store campaign identifiers — click IDs such as gclid or fbclid, UTM parameters, the original landing URL, and the referring site — in a first-party cookie and browser storage on this domain for about 90 days. That lets us credit the advertisement that brought you here even if you return later without those parameters in the address bar. It does not include answers from the appointment form or any health information.
When you submit the form we may share a hashed, irreversible version of your email address and phone number with these advertising partners so they can measure whether their advertising worked. Hashing converts the value into a fixed string before it leaves this page; the partners cannot read the original from it. This is used for measurement, not to build a profile of your health.
You can limit this through your browser’s cookie settings, your device’s ad-tracking controls, and the ad-preference settings offered by Google and Meta.
